New Patient Paperwork
This is intake paperwork that all new patients fill out. You can download, print, fill out, and return this paperwork at your first visit. You can also fill it out, scan it to your device, and email it back to us at Contact@CacheValleyChiro.com. Alternatively, you can fill out this information in your Jane patient portal if you scheduled your appointment online.
New Patient Appointments
All new and "inactive" patients receive this appointment.
On your first visit to our office, you will have a new patient appointment which includes a full history and examination of your complaints. If you have been seen before but it has been two years since your last appointment, you will also undergo a new patient appointment. Treatment (adjustments, etc) usually happens on the first visit if time allows. This allows your physician to evaluate all of your complaints and ensure that the treatment we offer is safe and effective for your issue.
A new patient appointment lasts about 50 minutes and costs $110 unless you are a Medicare patient. Please inform the office staff if you are a Medicare patient so that you may be quoted appropriate fees.
